Data Request Instructions

1. Navigate to  and logon using your GGC credentials and MFA protocol.
2. Select “Service Request” from the menu options at the top.
3. Select “Data Request” from the services list on the left.
4. Select “Data Request Form.”
5. Complete the form, and review the GGC Confidential Data Agreement.
